Lakeland offers an array of specialized home-based services to help individuals maintain or improve health. Rely on our team of skilled professionals to provide the same comprehensive, high-quality, compassionate care you’ve come to expect from Lakeland.

Our skilled homecare nurses work closely with physicians to ensure that patients receive exactly the care they need. We provide care to patients of all ages and have specialists in pediatric and geriatric care. Services may include:

  • Care management and assessment
  • Disease and treatment education
  • Dressing changes
  • IV management
  • Management of catheters
  • Medication administration
  • Medication reconciliation
  • Reports to physician

Home Health Aides support the goals of the care team and may assist with:

  • Ambulation
  • Bathing and hygiene

The Veggie Van™, a mobile farmers market, served 13 counties throughout Michigan this year, ensuring that free fruits and vegetables were available to individuals who otherwise had limited access. The program, a joint effort of Corewell Health™ and the YMCA of Greater Grand Rapids, launched in 2018 to combat food insecurity and will continue for a sixth consecutive year in spring 2024. Through their partnership, 46,466 free pieces of produce were distributed to Michigan families in 2023.
